What to Bring to Your Appointment
- Photo ID (driver's license or state ID)
- Current insurance card
- List of all current medications and dosages
- Completed patient forms (see our Forms page)
- Records or test results from other providers (if relevant)
- A list of questions or concerns you'd like to discuss
Becoming a PHA Patient
Welcome! We're glad you're considering Primary Health Associates. To establish care with one of our physicians:

1Choose a physician - Visit our Physicians page to learn about each of our doctors and their locations.

2Call their office directly - Each physician manages their own scheduling.

3Verify your insurance - Contact your insurance carrier to confirm in-network participation before scheduling.

4Complete new patient forms - Download from our Forms page and bring them with you.
Requesting a Refill
For prescription refills, please contact your physician's office directly during business hours. Be sure to have the following information ready:
- Medication name and dosage
- Your preferred pharmacy and its phone number
- Approximate date of your last refill
Please allow 24–48 business hours for refill requests to be processed.
When We're Closed
For medical emergencies, call 911 or go to the nearest emergency room.
For non-emergency concerns after hours, call your physician's office. The voicemail will provide instructions for reaching the on-call physician if needed.
Requesting Medical Records
To request a copy of your medical records or to have records transferred to another provider, please download and complete the Authorization to Release Medical Records form from our Forms page. Submit the completed form to your physician's office.
